Document Requirements
The following documents are required to complete the home study (preplacement assessment). You will hand these documents to your social worker during the meetings. You do not have to gather these
documents before the first meeting, but your social worker must have them all in order to write your final home study.
During the interviews, the social worker will review this list with you. Please keep your originals, all documents submitted must be photocopies.
- Copies of Birth Certificates for each member of the household, including children, as well as those who do not
live in the household.
- A copy of your Marriage Certificate issued by the state.
- A copy of divorce decrees for ALL previous marriages of both spouses.
- A copy of the first two pages of your most recent 1040 tax form. In addition, we need either a pay stub or a
letter from your employer(s) verifying employment.
- Criminal Record Checks from all counties in which you have resided for the past three years.
- Medical Clearances for all members of the household (including children).
- Three letters of reference - notarized. Please select references
who are not family members and know both prospective adoptive parents.
