Home Study Application Process

1. Please read the “Dear Prospective Adoptive Parents” letter carefully, print it out, and save it for your files.

2. Complete the following required documents :

Application – please print, complete in full, and sign.
Responsible Individuals List (RIL) – please print one form for each person age 17 and over in the home, TYPE the required information in the Applicant Information section on the top right hand side, print, sign/date the Applicant Acknowledgement section.
Fee Agreement – please print and sign.

3. Send us your completed documents and submit payment by:
